General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees operating a freight elevator and/or riding a freight elevator, and/or working in the vicinity of a freight elevator cab due to the elevator hoistway door not having functioning interlock system: a) Location: First Floor; The existing electrical interlock was bypassed using a piece of wood which was wedged between the interlock and door guide. The condition had existed for approximately one month. Violation observed on or about 9/11/91 Among other methods one feasible and acceptible abatement method to correct this hazard is to: 1. Equip the hoistway door with a working interlock device. 2. The interlock device shall hold the door in the locked position by means of gravity or by a restrained compression spring, or by both or by means of a positive linkage. 3. The interlock shall lock the door in the closed position before the elevator can be operated.
