Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the potential hazard of a toxic unknown atmosphere in a confined space: 1) Eyelet Department - Bulk Degreaser; On 9/22/90 One employee was exposed to the hazardous condition associated with entry into the confined space of the degreaser unit that measures 51 inches wide by 8 ft. long by 8 ft. deep and was contaminated with residual trichloroethylene. Deficiencies in the plan confined space entry procedure contributed to the hazardous exposure. These deficiencies included: a) The entry permit issued did not include all of the necessary information (Reference ANSI Z117.1-1989, Section 5.1.1,5.1.2, 5.1.4, 5.1.5, 5.1.6).
