Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing/amputation hazard during maintenance and set-up procedures due to inadvertant stroke or activation of mold, box machines and scrap grinders: a) Graham Container - Eight (8) mold machines, box machines and scrap grinders, no name or serial number are worked on, and equipment can be inadvertently started during maintenance and set-up, on or about March 12, 1987. Among other methods, one feasible and accepted abatement method is to implement a lock out-tag out procedure.
