Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to injury from failed chain slings and the resulting falling load. a) Truck driver employees at the Riverside Facility used a high carbon steel chain as a sling for the overhead lifting of sections of steel rails. The chain was not used in accordance with the safe usage recommendations of the chain manufacturer or of industry practice, 2/19/91.
