Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to multiple fractures due to a 15 foot fall hazard:
The alleged violations below have been  grouped  because  they  involve
similar or related hazards that may increase the potential  for  injury
resulting from an accident.
a)  Chandlers Crossing - Two employees were hoisted from
a Lull Model 622 Rough Terrain forklift with a homemade
work platform attached to the forks of the truck.  Employees
were observed placing trim on a building approximately 15
feet from the ground, standing on platform with use of fall
protection or standard guardrails.  The operator manual
for the Lull 622 strictly forbids the use of the machine
to lift personnel and as a work platform, 7/1/91.
Among others one feasible and acceptable abatement method to correct
this hazard is to access this elevation by using a properly constructed
scaffold or through the use of a machine specifically designed for
elevating personnel.
