Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fractures and multiple contusions from falling loads: A) Shop and Yard Area: Employees use lifting bars/devices to lift precast concrete castings and forms weighing approximately approximately 300-14,000 pounds. The tube spreader bar, angle lifting bar, 96" seepage ring bar, and the bar lifting capacity, on or about September 25, 1990. B) Plant #2 and #3: Employees use carbon chain slings to lift material weighing approximately 300-6000 pounds. The chain slings are not being used in accordance with the safe usage recommendations of the device manufacturer, on or about September 25, 1990. Among other methods, one feasible and acceptable method to correct this hazard is to: 1) Implement a below-the-hook lifting device safety program as described in ANSI B30.20-1985. This program should include at a minimum: A) Marking, construction and installation B) Inspection, testing and maintenance, and: C) Operation requirments for structural/mechanical and remotely operated lifting devices, etc. (Instance A) 2) Replace the carbon steel chain slings with alloy type slings. A safety program complying with the requirments of 1910.184(e) for alloy stel chain slings should be implemented.
