Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard due to possible chain failure: (a) Alpo Pet Food - The employer was hoisting material overhead with a 3/8 inch carbon steel chain sling 16 feet long, on or about April 26, 1991. Among other methods, one feasible and acceptable abatement method to correct this hazard is to use alloy steel chain slings for hoisting overhead. The alloy steel chain slings shall be maintained with monthly inspections and the chain slings shall have tags that identify the size, grade, rated capacity and reach for each chain sling.
