exposed to a crushing hazard:

The back-up alarm was disconnected on the Caterpillar DGH
dozer, Serial Number 4RC00447 and the spotter was not alerted
to location of dozer.
ABATEMENT NOTE:
Among other methods, one feasible and acceptable abatement method
to correct this hazard is to make sure the alarm is always connected
and in operable working condition.
