General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of ingoing nip points on the #1 cushion mill in the Tread Rubber Department when the rolls were inadvertently operating in reverse. The electrical switch which controls the direction of rotation of the rolls was not appropriately labeled to indicate its purposes. Also, some employees were not aware of the presence of this switch. Among other methods, one feasible and acceptable abatement method to correct this hazard is to appropriately label this switch, as to what it controls, and also what is the forward position and what is the reverse position. Procedures should also be developed to ensure that before this machine is run, the switch is in the forward
