Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hydrogen cyanide from the inadvertent admixture of acid and cyanide and inadvertent admixture of sodium hypochlorite and acids. a) Bright Dipping Department, Tubbing Room - Procedures for the safe storage and handling of sodium cyanide were not established. The following are instances in which cyanides and acids were stored together potentially exposing employees to hydrogen cyanide gas: 1) An open bucket containing sodium cyanide solution and an open bucket containing sodium cyanide pellets were stored adjacent to the hood, where the acid bright dipping operation is performed. The acid bright dip contains sluphuric and nitric acids. 2) A ceramic pitcher used to transfer spent acid bright dip, containing sulphuric and nitric acids was stored next to an open bucket containing sodium cyanide solution. 3) A 100 lb. drum of sodium cyanide pellets was stored adjacent
