Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Employees exposed to hazard of fire/explosion - in storage building a clear space of at least 2 feet was not maintained between organic peroxide storage and uninsulated metal walls. Among other methods, one feasible and acceptable abatement method to correct this hazard is to maintain at least a 2 foot clear space between MEKP containers and uninsulated metal walls. b) Hazard of fire or explosion because a minimum 8 foot clear space was not maintained between organic peroxide storage and any other storage. Among other methods, one feasible and acceptable abatement method to correct this hazard is to maintain at least an 8 foot clear space between MEKP storage and any other storage (NFPA 43B, 1986 3-4,1).

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free form recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) A fire hazard in the Laminating Building: All spraying operations involving the use of organic peroxides such as MEKP and other dual component castings were not conducted in approved sprinklered spray building. One feasible and acceptable abatement method is to install an approved sprinkler system in the spray building.
