Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to an anhydrous ammonia hazard:
a.On or about April 21, 2000, anhydrous ammonia transfer area, the liquid
filling
connection at the globe valve did not have an internal back-pressure check
valve
or an internal excess flow valve, as required by American National
Standards
Institute (ANSI) K61.1-1989 section 5.5.11.
b.On or about April 21, 2000, anhydrous ammonia transfer area, the vapor
connection at the globe valve did not have an internal excess flow valve
as,
required by American National Standards Institute (ANSI) K61.1-1989 section
5.5.11.
Among other methods, one feasible and acceptable method to correct this
hazard
is to install an excessive flow valve for both the liquid and vapor
connections at
the respective globe valves.
ABATEMENT CERTIFICATION IS REQUIRED.

not ensure that an employee wore protective gloves and chemical splash
goggles
before transferring ammonia, as required by American National Standards
Institute (ANSI) K61.1-1989 section 3.2.1.
Among other methods, one feasible and acceptable method to correct this
hazard
is to ensure that all employees exposed to anyhdrous ammonia under normal
conditions wear protective gloves and chemical splash goggles.

employer did not provide emergency and rescue equipment such as, but not
limited to protective boots and protective pants, as required by American
National
Standards Institute (ANSI) K61.1-1989 sections 3.4.1.3 and 3.4.1.4.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard is to provide and maintain one pair of protective boots and
pants.
This personal protective equipment (PPE) must be on hand at the permanent
storage installation.
ABATEMENT CERTIFICATION IS REQUIRED.
