furnish employment and a place of employment which were free from
recognized hazards
that were causing or likey to cause death or serious physical harm to
employees in that
employees were exposed to electric shock hazards when they were permitted
to work inside
Altec aerial lift buckets with materials, debris, tools and equipment on
the floor of the bucket
which could potentially compromise the insulation integrity of the bucket
liner.
Among others, one feasible and acceptable method to correct this hazard is
to comply with
ANSI A92.2-1990, Section 8.2.3.  "Responsibilities of the Owners and
Users, Frequent
Inspection and Test" and the manufacturer's instructions to inspect the
bucket prior to use
and to require employees working inside of it to remove materials, debris,
tools and
equipment from the floor of the bucket before working inside of it.
a.On August 6, 2000, employees did not inspect the bucket lining of an
Altec aerial lift,
for cuts, cracks and defects before working inside of it and began working
inside of it
with several items of metal materials, debris, tools and equipment
(including but not
limited to copper wiring, a cable/wire cutter, metal bolts and nuts, metal
clamps and
brackets, and metal eye bolts) which could damage the integrity of the
bucket lining.
