Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Employees using the "C" hook coil lifting device to lift steel coils with the overhead crane in the coil storage area were exposed to the hazard of being struck by falling loads in that: A. The lifting eyes on the "C" hook were worn / defective. B. The "C" hook's identification plate bearing it's weight and serial number was painted over, and the weight was unknown. C. The "C" hook was not being inspected on a regular basis. Among others, one feasible and acceptable method to correct this hazard is to ensure that the provisions of ANSI/ASME (American National Standards Institute / American Society of Mechanical Engineers) B30.20 safety standard for "Below the Hook Lifting Devices" relative to marking, inspection, maintenance and repair of the "C" hook are met.
