Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: a)On or about December 17,1999, employees were exposed to the hazard of being struck by bolts and lifting lugs due to the improper thread engagement at one of the four lifting locations during the lift of a 750 ton WP SF machine press. Among other methods or feasible and acceptable abatement methods to correct this hazard is: 1)Comform to the ANSI/ASME B18.3-1960 Series Socket Head cap screw thread engagement specification. 2)Add a backing plate and nut on the back side of lifting lug bolts and torque to specification.
