Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Employees were exposed to the hazard of point of operation injuries on Press Brake #9612528, in that persons responsible for ensuring safe set-up were not trained in proper procedures for setting the stop / stroke devices, proper positioning of light curtains and the object sensitivity functions of the light curtains. One feasible and acceptable method of abatement is to train employees set-up personnel in accordance with American National Standard Institute "ANSI" B11.19.
