Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a  place  of  employment  which
were free from recognized hazards that were causing or likely to  cause
death or serious physical harm to  employees  in  that  employees  were
exposed to the hazards of falling into  the  hoistway  shaft  when  the
elevator car was not in the landing zone  on  the  second  floor,   and
employees were also exposed to walking underneath the elevator,  on the
first floor.   Among other methods,  one feasible and acceptable abate-
ment method to correct the first  hazard  is  to  provide  an  elevator
hoistway door or gate that is electrically interlocked so that  latched
doors or gates cannot be opened from the landing,  unless the  elevator
is at that level.   A possible abatement for the  second  hazard  would
be to block off or baricade the first floor level to prevent  employees
from being able to walk underneath the elevator and have that  gate  or
barricade also interlocked.
a) 2nd Floor - A means was not provided to prevent employee
exposure to falls of up to sixteen feet to the 1st floor,
in that the sliding doors on the 2nd floor could be opened
without the elevator car being at the landing.
b) 1st Floor - A means was not provided to prevent employees
from walking underneath the elevator, when the elevator
was in the raised position.
