Section 5(a)(1) of the Occpational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of a building structure failure due to dislodgement and
failure of the jacks:
a) Lifting operation of the south section: On or about 12/3/87, the
lifting jacks were not secured to the columns to preven dislodgement
of the jacks if a component, such as a rod, failed.
Among other methods one feasible and acceptable abatement method to
correct the hazard would be to secure the jack to the column in such
a manner that a faillure of any component of the jacking system would
not result in the dislodgement of the jack.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to the employees in that employeed were exposed
to the hazards of a building structure failure while working/walking
under floor slabs which were suspended on the lifting system:
a) Lifting operation of the south section: On or about 12/3/87,
employees not essential to the jacking operation were permitted
to work under slabs while the slabs were suspended on the lifting
system when provisions were not adequate to prevent a local failure
from precipitating a progressive failure of the building or any part
of the building.
Among other methods, one feasible and acceptable method to correct these
hazards is to assure that no employees, except employees required to
place a support system, shall be permitted to work/walk under floor
slabs being lifted or suspended until the floor slabs are adquately
secured in place.
