Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hydrogen cyanide from inadvertant admixture of acids and cyanides. a) Mixing Room adjacent to Tank Farm: Powdered cyanide that was stored in a cardboard container could intermix with concentrated liquid phosphoric acid that was stored in a plastic container less than three feet away. A mixture of these two chemicals could result in employees being exposed to a build-up of hydrogen cyanide gas.
