Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards due to overpressurization of a receiver vessel, caused by a runaway polymerization of methacrylic acid and styrene monmers on or about October 20, 1990. Production Area- A runaway polymerization reaction of methacylic and styrene monomers occurred when excessive steam was applied to the vessel during and subsequent to maintenance operations on the steam piping system. The following deficiencies were noted at the time of the inspection: a) Temperature and pressure sensors connected to alarms were not installed on the monomer receiver to immediately warn of sudden increases in receiver temperature and/or pressure.
