Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burn hazard from steam leaks: A) Steam pipes located in the boiler room, compounding department, RPC#2, dye house and maintenance shop have leaks and were not insulated or otherwise quarded to prevent contact by employees. Among other methods, one feasible and acceptable abatement method to correct this hazard is to insulate or otherwise quard the steam pipes so as to prevent possible accidental employee contact.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to the hazard of being in the hoistway shaft when the Pilot Range elevator car was not in the landing zone and being struck by the elevator car when the car was entering the landing zone. A feasibleand useful method to correct this hazard, among other methods is to provide electrically-interlocked elevator hoistway doors or gates to prevent employees from having any part of their bodies in the hoistway except when the elevator car is stationary in the landing zone.

SECTION 5(A)(1) OF THE OCCUPATIONAL SAFETY AND HEALTH ACT OF 1970: THE EMPLOYER DID NOT FURNISH EMPLOYMENT AND A PLACE OF EMPLOYMENT WHICH WERE FREE FROM RECOGNIZED HAZARDS THAT WERE CAUSING OR LIKELY TO CAUSE DEATH OR SERIOUS PHYSICAL HARM TO EMPLOYEES IN THAT EMPLOYEES WERE EXPOSED TO THE HAZARD OF UNAUTHORIZED PERSONS IN THE MACHINE ROOM OF THE MAINTENANCE SHOP'S INCLINED ELEVATOR BEING EXPOSED TO ACCIDENTAL CONTACT WITH ENERGIZED ELECTRICAL PARTS AND BEING EXPOSED TO MOVING PARTS OF THE INCLINED ELEVATOR, INCLUDING THE POSSIBILITY OF BEING STRUCK BY THE ELEVATOR CAR ITSELF. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD IS TO LIMIT ACCESS TO THE MACHINE ROOM TO AUTHORIZED PERSONS ONLY BY PROVIDING A DOOR WHICH IS SELF-CLOSING AND SELF-LOCKING BUT OPENABLE FROM THE INSIDE WITHOUT A KEY. THIS DOOR WOULD BE CLOSED AND LOCKED WHEN NOT IN USE AND WOULD BE PROVIDED WITH A SIGN RESTRICTING USE TO AUTHORIZED PERSONS ONLY.
