were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
a.   A crushing hazard created by the sudden and
unpredictable
descent of vehicles elevated overhead by
Gilbarco Model 119308-02 Hydraulic overhead
automotive
lifts.
a)   Service area, bay 1, automatic locking gear
mechanism rendered inoperative by the
placement
of a nut and bolt in the cover plate.
b)   Service area, bay 3, automatic locking gear
mechanism rendered inoperative by the
placement
of a wooden wedge to block its movement.
c)   Service area, bay 4, automatic load locking
toggle plate rendered inoperative through
failure to maintain in a clean condition.
d)   Service area, bay 6, automatic load lock
device was missing.
e)   Service area, bay 9, automatic load locking
mechanism rendered inoperative by the
placement
of an "oil seal" so as to prevent
actuation
of the device.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is to:
1.   Install and/or repair lock assemblies.
2.   Establish and implement weekly inspections.
3.   Instruct all employees in the proper use of safety
devices and ensure their routine use.
4.   Post manufacturer's operating instructions.
