Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
a.   A crushing hazard created by the fall of the load
elevated overhead by 1 ton electric hoist.  With
kinked wire rope that could fail.
a)   Maintenance shop, electric one ton hoist in
use with unauthorized and untested shop made
hook and visibly damaged wire rope.
Among other methods, one feasible and
acceptable
abatement method to correct this
hazard is to:
1.   Remove shop made hook.
2.   Post operating instructions.
3.   Train employees in proper inspection procedures
as autlined in ANSI B30.16 - 1973, Section
162.2.1.2.

4.   Restrict all modifications to those approved
by the manufacturer as per ANSI B30.16-1973
section 16-2.1.2.1(c).
5.   Implement an inspection program.
