Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to the hazard of electrical shock, burn, or
electrocution, where exposure to the potential use of ungrounded
appliances in the presence of water occurred. The following
conditions contributed to the existence of these hazards:
(a)M/V Aleutian Falcon, men's bathroom facilities:
Electrical outlets located directly above sinks were not
protected by ground-fault circuit-interrupters (GFCI) at
either the outlet or the electrical panel.
(b)M/V Aleutian Falcon, galley: Electrical outlets located
directly above the sinks were not protected by GFCIs at
either the outlet or the electrical panel.
NOTE: One feasible means of abatement, among others, is
to install a receptacle or circuit breaker that is
protected by a GFCI at each location.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to the hazard of electrical shock, burn or
electrocution where the potential existed for employees to be
unable to quickly de-energize an electrical circuit in the event of
an emergency. The following condition contributed to the existence
of this hazard:
(a)M/V Aleutian Falcon, lower level engine room: Where the
distribution panel labeled "208/120V" was  energized and
indicated that all circuit breakers were in the "on"
position, not all of these panelboard circuits were
identified as to purpose or use on the circuit directory.
NOTE: One feasible and acceptable method of abatement,
among others, is to trace and identify each circuit on
the face or inside of the panel door.
