Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by a ruptured diverter
(washington head):
(a)Cannon Oil & Gas, Rio Blanco County; Meeker, CO:  On or before 12/21/02
the employer did not ensure that the employees followed the step by step
operating procedure when conducting the "landing the well" procedure.  In
accordance with API RP-54 5.1.3, employees should receive instruction in
correct
work methods to reduce chance of injury to themselves or fellow employees.
A
safety program should be established and maintained.  Ineffective employee
training exposed employees to a struck by hazard due to overpressurization
that
resulted in an amputation.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard would be to ensure that employees are educated and adequately
trained in
recognition, avoidance and prevention of unsafe conditions at the jobsite.
