Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of severe lacerations and finger amputations during cleaning, maintenance and blade changes involving the header machines: a)F/V Lilli Ann, processing area - Where engineering and processing personnel were required to clean, maintain, and perform blade changes on the header machine at this location, workers exposed themselves to the hazard of severe laceration and finger amputation from contact with the sharpened blade energized by pneumatic sources. The energy isolation program in place consisted only of an understanding of process and did not provide written guidance with specific steps for shutting down and isolating equipment nor did it cover the placement, removal, and transfer of lockout/tagout devices; machine specific procedures were not developed for testing, verifying the effectiveness of these devices, and other control measures such as pressure bleed and valving.
