Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employment at the Ward Manufacturing Company, 115 Gulick Street, Blossburg, PA, Plant #1, Cupola and Snible, Air Pollution Device, were exposed to hazards by entry into a space of limited size and accessibilty from which safe removal would be difficult in the event employees recieved serious injury or became ill. a) Standard operating procedures for confined space entry during cleaning operations of the Snible and the Cupola were not posted in the workplace, October 16, 1989. b) Employees were permitted to enter the Cupola and Snible to loosen and remove material from sides of Cupola and Snible without readily available emergency equipment consisting of a safety line of sufficient strength, a harness (or wrist cuffs) so that an employee could be rescued in an expeditious manner, October 16, 1989. c) Provisions for a standby employee outside the Snible or Cupola who was capable of monitoring what transpired inside the Snible and/or Cupola and was capable of removing the worker and or obtaining immediate help in case of an emergency, October 19, 1989. d) An emergency alarm and/or reporting system for obtaining immediate help for employees removing built up material from the sides of the Cupola and Snible was not readily available in case of an emergency, October 16, 1989. AMONG OTHER METHODS SOME FEASIBLE AND ACCEPTABLE ABATEMENT METHODS TO CORRECT THIS HAZARD ARE: 1) Do not permit employees to enter Snible and/or Cupola without written procedures, training, etc.; 2) Provide safety equipment for employees if necessary to enter the Cupola and Snible; 3) Post confined space procedures at work area and instruct employees in the rules and regulations; 4) Provide a standby employee outside on top of Snible and/or Cupola in case of an emergency; 5) Provide communications and/or alarm systems at Snible and/or Cupola for use in case of an emergency.

Section 5(A)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a falling hazard when working from an inadequately guarded metal cage in the elevated position by a powered industrial forklift: a) Weld Manufacturing Company, 115 Gulick Street, Blossburg, PA 16912, Plant #1, Cupola Area: employees when working from an inadequately guarded metal cage (64"x36"x36") elevated in the air by a powered industrial forklift and the cage was not securely attached to the forks of the truck, exposing employees to a falling hazard and possible serious injuries. October 16, 1989. AMONG OTHER METHODS, SOME FEASIBLE AND ACCEPTABLE ABATEMENT METHODS TO CORRECT THIS HAZARD ARE: 1) Provide adequately and properly guarded work platform (cage); 2) Platform must have a slip resistant surface; 3) Platform shall be securely fastened to the fork of the industrial truck; 4) An operator shall be in control position of the powered industrial truck at all times; 5) Means shall be provided to protect personnel in the normal working position from moving parts of the truck that represent a hazard; 6) Provide training for all personnel.
