Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
crushing injuries and/or hands:
a) The foot pedal mechanism used to activate the Verson No. B-710 press
brake in the Machine Area was not protected to prevent unintended
operations from falling or moving objects or accidentally stepping
onto the pedal. The foot pedal was not provided with a non-slip
surface, September 13, 1989.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO
CORRECT THIS HAZARD IS:
1) The installation of a pedal guard (cover) to prevent unintented
operation from falling or moving objects or accidentally stepping
onto the pedal. Attach a non-slip surface to the foot pedal.
