Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a falling hazard to the concrete floor below when working on an unguarded wooden work platform elevated approximately 0' to 25' in the air by powered industrial forklifts: a) Warehouse Area, employees working of an unguarded wooden work platform (45" x 48" x 65") elevated to various heights by powered industrial forklifts to retrieve materials from steel storage racks, and the platform was not secured to the forks of th trucks, were exposed to a falling hazard to the concrete below and possible serious injury, October 4, 1989. AMONG OTHER METHODS, SOME FEASIBLE AND ACCEPTABLE ABATEMENT METHODS TO CORRECT THIS HAZARD ARE: 1) Provide proper and standard guardrail and toeboard on platform, top rail not less than 39" nor more than 45' from surface of platform; 2) Platform must have a slip resistant surface; 3) Access opening to platform through guardrail must be equivalent to that provided by guardrail; 4) Platform shall be securely fastened to the forklifts; 5) An operator shall be in control position of the powered industrial truck; 6) Means shall be provided to protect personnel, in the normal working position, from moving parts of the truck that represent a hazard; 7) Provide training for all personnel.
