or serious physical harm to employees in that foot pedal mechanisms on
machinery were not protected to prevent unintended operations from
falling or moving objects or by accidentally stepping onto the pedals:
a)  The foot pedal (treadle) on the Verson #36 Press
Brake was not guarded, September 23, 1992.
b)  The foot pedal (treadle) on press brake #13 was
not guarded, September 23, 1993.
c)  The foot pedal on the Semco #31 Press Brake was
not guarded, September 23, 1992.
d)  The foot pedal on the Verson #16 Press Brake was
not guarded, September 23, 1992.
e)  The foot pedal on the Verson #35 Press Brake was
not guarded, September 23, 1992.
f)  The foot pedal on the Verson #15 Press Brake was
not guarded, September 23, 1992.
