Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to a fall hazard:
a)  Employees were exposed to fall hazards while
climbing a rebar frame 14 feet high to gain access
to the inside of the Administration Building area,
to secure a bulk head, October 15, 1992.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE
ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO PROVIDE
A LADDER ON EACH SIDE TO ASCEND AND DESCEND THE WALL.
b)  Employees were exposed to fall hazards while
traversing the top of the south wall of the pipe
gallery to get to the area of the staging scaffold,
October 15, 1992.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE
ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO PROVIDE
TEMPORARY GUARDRAILS ALONG THE OPEN SIDES.
c)  Employees were exposed to fall hazards while
traversing the top of a concrete wall to leave one
ladder and get onto another ladder to enter and exit
from the floor of the Pump Station, October 15, 1992.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE
ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO
CONSTRUCT TEMPORARY GUARDRAILS ALONG OPEN SIDES.
