Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees using single point suspension systems were not attached to lifelines rigged independently of the primary support system: a) 2nd & Chestnut ST, Customs House, Phila, Pa. 19103 - two employees attached lifelines to same anchorage point as primary support system and were not connected to the lifelines. One employee had no lifeline. Observed 4/11/91 NOTE: One useful and feasible means of abatement, among others, would be to ensure that all workers are attached to lifelines rigged separately from primary support lines when using single point suspension systems.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees had not determined if the roof attachment points used to anchor the primary support system and lifeline of a single point suspension system were capable of supporting the loads imposed on it: a) 2nd & Chestnut ST., Customs House, Phila, Pa. 19106 - employees attached their primary support and lifeline to the railing at the edge of the 4th floor roof. Observed 4/11/91 NOTE: One useful and feasable means of abatement, among others, would be to load test the attachment points for the primary support and lifelines prior to the start of the job.

Observed 4/11/91 NOTE: One useful and feasible means of abatement, among others, would be to develop and implement a rescue system which does not rely on actions of the suspended employee.
