a) Iron and Transportation Division
1) Blast Furnace Department - Coke Breeze loading-out station - The
entire structure had severe structural damage including; damaged
base structures, cross-bracing angles missing and steel members
corroded, observed August 8, 1989.
2) Blast Furnace Department - Junction House for #9 and #10 conveyor,
the entire structure had angles missing and steel members severely
corroded, observed August 8, 1989.
Among other methods, one feasible and acceptable abatement method to
correct this hazard would be to correct all structural deficiencies as
noted in the in-house structural survey conducted by Orbital
Engineering.

head structures due to the severe corrosion and deterioration of the
roof structure:
a) Iron Transportation Division
1) Sinter Plant Department - There were several areas of the Sinter
Plant roof that were severely deteriorated and in danger of
collapse. Steel sheeting sections that collapsed in some areas
pieces dangling overhead above active work areas, observed August
8, 1989.
Among other methods, one feasable and acceptable abatement methodto
correct this hazard would be to correct all structural deficiencies as
noted in the in-house structural survey conductedby Orbital
Engineering.

slips, trips, and falls due to insufficient illumination provided
at Open Hearth employee walkway to parking lot. Illumination
readings indicated from 0.1 to 4.0 footcandles of lighting,
observed September 13, 1989.
2) Open Hearth Department - Employees were exposed to the hazard of
slips, trips, and falls due to insufficient illumination provided
at steps down from Open Hearth #8 and #9 furnaces. Illumination
readings indicated 0.1 footcandles of lighting, observed

3) Open Hearth Department - Employees were exposed to the hazard of
slips, and falls due to insufficient illumination provided behind
#9 Open Hearth Furnace. Illumination reading indicated 0.5

a) Rolling Division
1) 45" Mill, Oil and Pump Cellar - The oil separation surge tank was
not provided with a pressue gauge on or in the near proximity of
the vessel to determine its operating pressure. An intervening
stop valve was installed between the pressure relief device and
vessel creating a potential hazard in the event the valve was in
the closed position, observed August 7, 1989.
2) 40" Mill, North End of Oil Cellar - Two oil surge tanks with an
operating pressure of 30 p.s.i.g. nominal, were not provided with
pressure relief devices and the nameplates on each vessel were not
legible, observed August 10, 1989.
3) 80" Mill, Coiler Area - The coiler oil surge tank with an
operating pressure observed at 22 p.s.i.g. nominal, had two
intervening stop valves installed between the vessel and the
pressure relief devices creating a potential hazard in the event
the valves were in the closed position. The system's pressure
relief devices that were located on the two oil pumps were not
marked or tagged with the required data, observed August 28, 1989.
Among other methods, one feasible and acceptable abatement method would
be to recognized and meet Commonwealth of Pennsylvania Act No. 451
(Boiler Law) and regulations governing unfired pressure vessels.

(mushroom type) palm buttons used to initiate stroke on the
Steelweld (large) shear, serial number M4850, located in the
Fabrication Department were not protected by ring guards (or the
equivalent). The shear blade was inadequately guarded, also,
observed August 15, 1989.
Among other feasible and acceptable methods to correct this hazard would
be to provide guards for the palm buttoms.

number M4995) to form metal. The foot pedal control is unguarded

1) Four and Five Stand - Employees must lift a 227 pound shim, one
side at a time, in preparation for installing new rolls, and an
evaluation of these manual lifting tasks indicates that employees
are exposed to an increased risk of musculoskeletal injuries based
upon studies of populations exposed to similar lifting conditions,
observed July 17, 1989.
Among other methods, one feasible and acceptable method to correct this
hazard is to employ the use of mechanical handling aids and/or the
redesign of working station heights.
Step 1: Effective administrative protection, such as employee training,
physical assessment, job rotation, etc. shall be provided as an
interim protective measure until feasible engineering or permanent
administrative controls can be implemented which will reduce employee
exposure to nominal risk.
Step 2: Submit to the Area Director a written, detailed plan of
abatement outlining a schedule for the implementation of engineering
or administrative measures to control employee injury due to manual
lifting tasks. Engineering controls to reduce musculoskeletal
stresses include, but are not limited to, job redesign, reduction
of the horizontal and vertical load distance from the body, reduction
of the lifting frequency or weight, and use of mechanical lifting
aids. This plan shall include at a minimum, target dates for the
following actions which must be consistent with the dates required
by this citation:
1) Evaluation of the conditions, locations and manual lifting
activities that the employees are performing at the onset of
musculoskeletal injuries.
2) Evaluation of applicable control measures.
3) Procurement, installation and operation of selected control
measures.
All proposed control measures shall be approved for each particular use
by a person trained in the evaluation of workplace conditions which
cause musculoskeletal disorders. Thirty day progress reports are
required during the abatement period.
Step 3: Develop a rehabilitation program for injured workers. The
program is expected to include exercise, building stamina, training
in safe lifting techniques, work conditioning and demonstration that
the worker is able to resume work.
Step 4: Abatement shall have been completed by the implementation of
feasible engineering and/or administrative controls.

liftng 7 to 15 ton coils, observed August 22, 1989.
2) 80" Mill - Slay Yard #2 - The four magnet lifting beam assembly
had one of its two main support bolts significantly backed out
of its nut creating a potential safety hazard at the #42 semi-
gantry crane while lifting slabs weighing 7 to 15 tons, observed
August 14, 1989.
3) 80" Mill - Slab Yard #1 - The Heppenstall (used on crane #34) 80
ton capacity automatic slab tong (four-point type) had six bolts
missing from keeper plates at pivot points and a cotter pin
missing from the loose "monkey hoist" bolt (bolt could be backed
off by hand) creating a potential safety hazard while lifting 5 to
6 slabs (each weighing 7 to 15 tons) at a time, observed August
11, 1989.
4) 80" Mill - Coiler Area - The Heppenstall (#4447) 40,000 pound
capacity vertical axis coil grab had one of two keeper plate bolts
backed out at a pivot joint creating a potential safety hazard
while lifting 7 to 15 ton coils, observed August 22, 1989.
5) 80" Mill - Slab Yard #2 - The Heppenstall (#45) 80 ton capacity
automatic slab tong (four point type) three loose keeper ring
bolts on the lower stabilizer beam with a bolt missing on keeper
ring located at the other end of the beam creating a potential
safety hazard while lifting 5 to 6 slabs (each weighing between
7 to 15 tons) at a time, observed August 14, 1989.
6) Central Maintenance Division - #2 Soaking Pit - The alloy chain
lifting beam assembly used to lift pallets of materials (e.g.
refractory, mortar) had one of the two lifting bars bent
significantly creating a potential safety hazard, observed July
31, 1989.
b) Sheet and Tin Division
1) Roll Shop #1 - The homemade spindle lifting device was not tested
before it was used to hoist 200 pound rolls onto the lathe,.
observed August 7, 1989.
2) C Hooks are used to hoist coils of steel weighing from several
pounds to 50,000 pounds. Numerous nuts were found to be loose on
the plates or lifting eye. Failure of these securing devices could
cause the load to drop 20-30 feet, observed June 28, 1989.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to implement a below-the-hook lifting device
safety program as described in ANSI B30.20-1985. This program should,
at a minimum, include:
1) Marking, construction, and installation;
2) Inspection, testing, and maintenance; and,
3) Operation requirements for structural/mechanical and remotely
operated lifting devices, etc.
