General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to fall and topple over hazard while working
ten (10) feet high:
(a)  Billet Department:  An employee was working on the work
platform of the Biljox Cougar lift and had not fastened the mid
rail chain, the out riggers were not extended and the breaks were
not applied.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is;
1.  Chains on the access opening shall be latched prior to moving
the lift;
2.  Brakes shall be engaged prior to entering the platform;
3.  Outriggers shall be fully extended prior to entering the
platform;
4.  All employees using elevating work platforms should be trained
in the following
A.  All safety equipment and instructions;
B.  All safety operation procedures;
C.  All safety maintenance procedures;
5.  All elevating work platforms shall be inspected prior to use at
each shift or with each new operator.
