employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
unintended activation and amputation or crushing from the press brake:
a) Building C - Machine number 228, Chicago Dreis & Krump press brake,
with operator's fingers 3 to 4 inches from danger zone had unguarded
foot pedal.
A feasible and useful method to correct this hazard is the fabrication
of a guard that would cover the foot pedal/treadle.
