Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
recognized hazards of being overcome by toxic vapors or oxygen deficient
atmosphere when cleaning the inside of storage tanks on 4/4/86, located
at Rockwell International Corporation in Carpentersville, Illinois.  The
employer did not determine and establish safe procedures to protect
employees entering such confined spaces.
Among other methods, one feasible and acceptable method to correct this
hazard is to establish and maintain a Confined Space Entry Program. Such
a program shall include, as a minimum, the following elements:
1) Written procedures which specify the permit system, ventilation
requirements, atmospheric tests, rescue procedure, and employee
training.
2) Warning sign and entry permit which document entry authorization,
location, type of work, pre-entry tests or air monitoring,
3) Mechanical ventilation prior to entry and continuously as determined
by tests.
4) Atmospheric tests prior to each entry for the presence of sufficient
oxygen (19.5% to 25%)and the absence of toxic or flammable
substances. Monitoring shall be continued when the work process or
other conditions could introduce a hazardous atmosphere.
5) Rescue procedures and equipment for rescue and communication,
including safety harnesses, lifelines, hoist, and positive pressure
self-contained breathing apparatus as appropriate.
6) Training of personnel who work in confined spaces; of standby
personnel who monitor and communicate with the worker; and of rescue
personnel. Training shall include review of hazards, rescue procedure
and use of test and protective equipment.
