Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) At the #1 freight elevator, Building Engineer and the elevator
operator were exposed to the hazard of falls through elevator shafts
when opening the freight elevator doors.  There was no effective
interlock to prevent employees from entering the elevator shaft when
the elevator car was not in the landing zone.  In addition, the
emergency access key was used as the standard operating procedure
to open the elevator door.
A feasible and useful method to correct this hazard, among others, is
set forth below:
Provide an electrical interlock to prevent employees from entering
the elevator shaft when the doors are ajar on another floor or the
elevator car is not an the landing zone.
