Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death of serious physical harm to employees in that employees were exposed to: Compana Ready Lease - Well #5: On or about March 17, 1988, the pumping tee was not secured from rotation during a well blow down operation exposing employees to the hazard of being struck by the rotating tee. One feasible and acceptable method of abatement would be to tie down or block the tee to prevent rotation.
