Section 5(a)(1) of the Occpational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
...the hazards of being burned due to being trapped on the fingerboard
on the drilling rig, located at the M. Omioteknumber 3 wellsite,
located in Section 31, Township 1 North, Range 3 East, Marion County
IL (near Kell, IL), during a rigfire without having a means of escape.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is the installation of an emergency escape means
running from the fingerboard to a location remote from the wellhole and
the drilling floor following rigging up of the rig mast, as stipulated
in paragraph 4.7.1 of the American Petroleum Institute RP54-1981
recommended practices for Occupational Safety and Health for Oil and
Gas Well Drilling and Servicing Operations.

or serious physical harm to employees in that employees were exposed to:
The hazards of being seriously injured due to being struck by flying
drilling mud and its associated high pressure hoses and/or piping from
failed connections in the mud supply line running from the Emsco
duplex mud pump to the drilling rig, located at the M. Omiotek number
3 wellsite located in Section 31, Township 1 North, Range 3 East,
Marion County, IL (near Kell, IL).
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to provide a positive means of securing the ends
of all flexible mudline connections with a safety chain or wire rope
during drilling operations, and to insure that such means are properly
installed following drilling rig setup and before the mud pump is placed
into operation, as stipulated in paragraph 8.14.3 of American Petroleum
Institute RP54-1981. Recommended Practices for Occupational Safety and
health for Oil and Gas Well Drilling and Servicing Operations.

or serious physical harm to employees in that employees were exposed to:
...the hazards of being struck by a flying steel pry bar and/or the
breakout tong on the rotary table of the drilling rig, located at the
M. Omiotek number 3 wellsite loacted in Section 31, Township 1 North,
Range 3 East, Marion County, Il (near Kell, Il), where an employee was
observed holding a pry bar in the latch mechanism of the breakout tong
while breaking a pipe joint.
Among other methods, one feasible and acceptable abatement, method to
correct this hazard is the repair of all makeup and breakout tongs with
properly sized parts so that the tong latch will function properly
during normal drilling operations, as specified in paragraph 8.10.6 of
American Petroleum Institute RP54-1981, Recommended Practices for
Occupational Safety and Health for Oil and Gas Well Drilling and
Servicing Operations.
