Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
...the hazards of being struck by falling drill collars due to an
unintentional release of the Type A drill collar elevators on the
number 3 drilling rig, located at the Kent number 4A wellsite,
located in Section 11, Township 3 South, Range 3 East, Jefferson
County IL (near Opdyke IL).
Among other methods one feasible and acceptable abatement method to
correct this hazard is to require the use of double latch elevators
such as those already in use for drilling pipe.

collars and other heavy objects due to unintended fouling of catlines
resulting from grooves on the off-side cathead on the number 3
drilling rig, located at the Kent number 4A wellsite, in Section
11, Township 3 South, Range 3 East, Jefferson County IL (near Opdyke
IL).
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to visually inspect the cathead for defects, such
as grooves and flat spots, before use. If such defects are found, the
cathead is to be removed from service and not used until repaired by
refacing and machining.
