Section 5(a)(1) of the Occupational Safety & Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: A fall hazard a) Warehouse and outside storage area. During interviews, employer and employees stated that employees were standing on forks of forklift truck raised to a height of approximately 17-18 feet to change light bulbs and retrieve parts on upper storage shelving without the use of an appropriate safety platform. Employees were exposed to the hazard of falling 17-18 feet to the concrete ground as fall protection was not provided. Violation observed on or about 03/05/97. Abatement Notes: Among others, one suitable method for abating this hazardous condition is 1) Utilize only platforms designed for use in elevating personnel with forklifts. These platforms must meet the requirements set forth in ANSI/ASME B56.1-1993. 2) Train all employees about the hazards of falls associated with improper elevating methods, and the proper application and use of safety platforms.n
