Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and/or caught between type hazards due to the improper use of rigging hooks. a) Construction site - #50 Peachtree Ave., East Hanover, N.J.; Four (4) hooks used as part of a rigging set-up to lift a 60 foot mobile house trailer were used improperly. The load was not centered in the base (bowl/saddle) of the hook in order to prevent point loading of the hook. Employee(s) who were guiding the trailer which was being lifted by the crane, were exposed to being struck by and/or caught between hazards if the hook(s) failed. One feasible and acceptable method of abatement is to use the hooks as required by the American National Standards Institute, B-30.10-1993, Section 10-1.3, paragraph (c) which requires that "a load shall be centered in the base (bowl/saddle) of the hook in order to prevent point loading of the hook". Violation occurred on or about 3/7/97. And/or in the alternative Section 5 (a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to employee(s) in that employees were exposed to the hazard of being struck by and/or caught between type hazards due to the improper use of rigging hooks. a) Construction site - #50 Peachtree Ave., East Hanover, N.J.; Four (4) hooks used as part of a rigging set-up to lift a 60 foot long mobile house trailer were used improperly in that they were subject to back loading stresses. Employee(s) who were guiding the trailer which was being lifted by a crane were exposed to the danger of being struck by and/or being caught between the materials and/or equipment had the hooks failed. One feasible and acceptable method of abatement is to use the hooks as required by the American National Standards Institute, B30.10-1993, section 10-1.3, paragraph (d) which requires that "hooks shall not be used in such a manner as to place a side load or back load on the hook". Violation occurred on or about 3/7/97.d

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by and/or being caught between type hazards. b) Construction site - Mobile house trailer & #50 Peachtree Ave., East Hanover, N.J.; On 3/7/97 an employee was struck by the end of a 60 foot long mobile house trailer which had been lifted by a hydraulic mobile truck crane. The accident occurred after the wire rope slinging method used applied excessive force to one of the trailers cross member(s) causing same cross member to break away from the trailer chaises' main frame. One (1) sling shifted causing the hoisted trailer to drop downward and backward. In addition, the rigging method used did not include positive attachment(s) which would adequately control the load, i.e. shackles or other positive method. One feasible and acceptable method of abatement is to control the load as required by the American National Standards Institute, B-30.9-1990, Section 9-1.9, section 9-1.9, paragraph (e) which requires that, "the sling shall be hitched or rigged in a manner providing control of the load". Violation occurred on or about 3/7/97.
