employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
The hazard of smoke inhalation, or other injuries from working in a
cooking operation where
the fire suppression system had not been serviced in excess of 6 months.
a) Upstairs kitchen;
Employees working in a kitchen cooking area were not protected from a
fire/smoke hazard due
to the fire suppression system not being serviced every six (6) months.
Reference New Jersey
State Fire Prevention Code 5:18-3.4(e)(3).  One feasible and acceptable
abatement method to
correct this hazard is to have the fire suppression system serviced every
six months.

fire alarm system repaired as soon as a problem occurred.

The hazard of being struck by a pressurized compressed gas cylinder if it
should be knocked
over and the valve break.
a) Maintenance storage area;
Employee working and walking in the area near an upright unsecured
cylinder containing helium
at 1350 psi, was exposed to a struckby hazard if the cylinder should be
knocked over and the
valve break.  Referrence New Jersey State Fire Code 5:18-3.24(c)(9)One
feasible and acceptable
abatement method to correct this hazard is to secure the cylinder to a
wall.
