Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a falling/crushing hazard: a) Dock Area, Door No. 28: Employer failed to implement a procedure to ensure that trailers are not removed from the dock door(s) before they are closed and ready to be pulled from the dock. Violation occurred on 1/27/98 "Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the current policy of the yard jockey or driver walking to the back of the trailer to make certain the door is closed or develop and implement one of the suggestions in company memo dated 9/11/95 Section 4, A through E."
