Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
a.  LTV Steel Company, 11600 S. Burley Ave., Chicago, Illinois - On or
about July 1, 1996,
one employee was fatally injured and another one was seriously injured
while working on the
roof of the lab building when they were struck by pieces of Fesco
hardboard insulation that fell
off the balanced pallet lifter as the lifter was being hoisted under the
hook of the Grove 35 ton
RT 65S crane.  The hardboards were not secured from falling from the
balanced pallet lifter.
A feasible and useful means of abatement includes, but is not limited to
the following actions:
1. Ensure that the load is well secured and properly balanced in the
lifting
device before it is
hoisted more than a few inches, as recommended by the American National
Standard Institute's
(ANSI) B30.15-1973;
2. Follow industry practices as outlined in D.E. Dickie's "Crane
Handbook", 1975, which states
that before making a lift ensure that "the load has been properly rigged
and well secured to
prevent movement or dislodgement of any part..."
3. Follow industry practices as outlined in D.E. Dickie's "Rigging
Manual", 1975, which states
that "materials and equipment being hoisted must be loaded and secured to
prevent any
movement which could create a hazard in transit".
