The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to:
a. On or about December 5, 1997, at the job site located at 3720 W.
111th St., Chicago, IL., an employee working in a Simon aerial lift
was not provided with a harness and lanyard to prevent falling out
of the basket to the ground.
A feasible and useful means of abatement includes, but is not
limited to the following action:
1. Ensure that all employees who work in an aerial lift are
provided with and use a safety harness and lanyard which is
correctly donned and used according to the manufacturers'
recommendations.
