Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish a place of employment which was free
from recognized hazards that were likely to cause death or serious
physical harm to employees in that employees were exposed to
explosions or other hazards related to improper boiler inspection
and maintenance.
a).  At the jobsite located at 210 N 10th, Omaha, NE, the
employer did not inspect or blow down the low water cutoff on
the Cleaver Brooks boiler daily under actual operating
conditions as per manufacturer recommendations. The low water
cutoff was being blown down monthly or more frequently
dependent upon chemical levels.
b).  At the jobsite located at 210 N 10th, Omaha, NE the
employer did not clean the low water cutoff on the Cleaver
Brooks boiler every six months as per manufacturer
recommendations. The low water cutoff is cleaned yearly unless
there is a failure in the system.
Among other methods, feasible and acceptable methods to correct the
hazard include but are not limited to the following:
a).  Follow manufacturer guidelines and have the low water
cutoff inspected and blown down daily under actual operating
conditions.
b).  Follow manufacturer guidelines and have the low water
cutoff removed and cleaned every six months.
