Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish a place of employment which was free
from recognized hazards that were likely to cause death or serious
physical harm to employees. Employees were exposed to explosions or
other hazards related to improper boiler maintenance and repair.
a). At the jobsite located at 210 N 10th, Omaha, NE, the
Cleaver Brooks boiler second low water cutoff was bypassed
thereby exposing employees to possible overheating of the
furnace pipes resulting in pipe meltdown and possible boiler
explosion.
Among other methods, feasible and acceptable methods to correct the
hazard include, but are not limited to the following;
a). Follow manufacturers guidelines and make sure a control or
interlock is never bypassed to make a boiler run.
b). Follow manufacturers guidelines and make sure safety and
interlock controls are operational at all times.
Abatement certification is required for this violation. The
abatement certification sheet is enclosed with the citation.
