Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
On the welding line at the side seamer welder in the facility at 700 S.
7th Streets, Beatrice NE -
The employer did not furnish a place of employment which was free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that the
employees were exposed to:  The hazard of being injured caused by being
struck by the table
used to raise the material on the table up for repositioning as evidenced
by the following instance
-
The hazard of being injured caused by being struck by the table.
Among other methods, one feasible and acceptable method to control the
hazard is to:
1)  Install pressure sensitive matting on the opposite side of the side
seamer from the operator.
This would interrupt the activation of the table while the workers are in
the hazardous area until
they have left the danger zone.
2)  Re-evaluate the process flow to determine ways in which to preclude
this situation from
happening in the future.
3)  Install a system in which the operator is constantly aware of the
location of all workers in
the hazardous area prior to activation of the table.
4)  Implement a retraining program to verify employees are following
proper procedures that
have been developed.
5)  Work with engineers to design a safer system to lift the tanks off of
the
side seamer.
Abatement certification and abatement documentation is required for this
violation. The
documentation should include written verification of abatement, applicable
measurements or
monitoring results, and photographs or videos which you believe will be
helpful. The abatement
certification sheet is enclosed with the citations.
