Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
Hoover Materials Handling Group, Inc. - Beatrice, NE - The employer did
not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that: Employees were
exposed to the hazard of being struck by falling Sure-Drain stainless
steel tanks.
Center C-Line Welding Station #2 - On August 12, 2005, an employee
preparing to weld on a
126 gallon tank was injured when the tank fell out the C-fixture and
struck him on the ankle.
Two other similar 126 gallon tanks also fell out of C-fixtures on the day
of the accident because
the tanks were not properly secured and the tanks were too small for the
C-fixtures.
Feasible means of abatement may include, but are not limited to the
following:
1) Welders should properly secure tanks in C-fixtures with chains that
remain attached
to hooks and ensure all safety precautions have been met before beginning
work as
outlined in ANSI Z49.1:2005 - 3.2.3.3.
2) All welding of smaller tanks, including the 126 gallon Sure-Drain
stainless steel tanks,
should be performed with vacuum manipulators rather than on the C-Line.
3) Purchase or manufacture appropriate size C-fixture type units that
would safely secure
the size of tank being welded and operate material handling units in
accordance with
manufacturers' recommendations and instructions as outlined in ANSI
Z49.1:2005 -
3.1.2.
Abatement certification and abatement documentation is required for this
violation. The
documentation should include written verification of abatement, applicable
measurements or
monitoring results, and photographs or videos which you believe will be
helpful.
The abatement
certification sheet is enclosed with the citations.
